You are entitled to five weeks annual leave pro rata each year. These should be taken between 1st of April and the 31st of March.
Please request your holidays at least 14 days in advance to give the call centre staff time to arrange alternative cover for your service user. Once completed, hit the ‘Submit’ button at the bottom of the form to send directly to the call centre; alternatively you can email the completed form to email@example.com.
Holidays not taken cannot be carried over to the next year. There will be no payment in lieu of holidays not taken.
